Your First Step to Optimal Health
The Intake Form & Consultations
Prior to the first visit you will complete a comprehensive intake form. The intake form can be mailed to you or downloaded from our website (see links in left sidebar). This form provides a template for you to outline your health concerns, and provides the doctors a thorough medical history. We ask that you bring the completed intake to your first appointment, as well as, copies of pertinent lab results.
During the first office visit you and your Naturopathic doctor will review the intake form together. At this visit, the doctors will also order appropriate lab work. At your second visit, approximately 1-2 weeks later, the doctors will discuss your lab results and personalize a treatment plan for you.
Office Visits
Office visits are based on a rate of $120.00 per hour. Your first and second appointments are approximately 1 to 1 1/2 hours, and follow-up visits range from 15-45 minutes depending on the complexity of your case.
Phone Consultations
Phone consultations are also based on a rate of $120.00 per hour. We offer phone consultations, for out-of-state and long-distance patients and local patients for whom a phone consultation is more convenient. Phone consultations offer a practical way to get the care you need without the travel.

Clarification of Treatment Plan
There is no charge for phone calls to clarify treatment plan instructions. However, if new conditions arise and case management is required, an office visit (or phone consultation) will be required and fees applied accordingly.
Payment
Payment is due in full at the time of the visit. Cash, checks and
major credit cards are accepted forms of payment.
Cancellation Policy
Summit Natural Wellness Center requires cancellations for scheduled appointments 24 hours in advance during regular business hours (Mon., Tues. Thurs. & Fri., 9 a.m.-noon and 1:30 p.m. to 6 p.m.). We reserve the right to charge for missed or cancelled appointments. Charges are based on the allotted time at the rate of $120.00 per hour.
Insurance
Most insurance coverage is limited to those states that offer licensure to Naturopathic doctors. Currently Ohio is not a licensed state, and insurance reimbursement for services rendered by a Naturopathic doctor is unlikely. However, as demand for complementary and alternative medicine increases, more insurers are providing coverage. It’s a good idea to call your insurance provider and inquire about Naturopathic medical coverage. Summit Natural Wellness Center does not bill insurance providers.
Nutritional Supplements
During the course of care, the doctors will recommend certain nutritional supplements as part of your treatment protocol. We offer high quality, professional- and physician-line supplements from our on-site dispensary. We provide this service to you as a convenience and assurance of quality control.
A visit to your Naturopathic doctor is worth the time & effort. Be an active participant in your health and let your doctor be your teacher. After all it is the patient, not the doctor, who ultimately creates healing.
